New Patient Forms

We want your time in our office to be as pleasant as possible. That said, your time in the waiting room should be kept to a minimum. To help make that happen, we’ve provided our new patient forms here so that you can print them from home, fill them out, and bring them with you to your appointment.

Why Choose Our Hearing Center?

At Southbury Audiology, we take a whole-person approach to hearing care. Our doctors of audiology are experts in diagnosing and treating hearing concerns for patients of all ages, including children. Rather than focusing solely on your ears, we consider your overall health to ensure you receive the most complete care.

For more than 20 years, we’ve supported the Greater Waterbury community with comprehensive evaluations and clear communication. We explain your results in a way you and your family can understand, and we keep your primary care or referring physicians informed when appropriate.

Depending on your needs, we may recommend hearing aids, assistive listening devices, specialty referrals, cochlear implant evaluations, or auditory processing strategies.
