New Patient Forms
We want your time in our office to be as pleasant as possible. That said, your time in the waiting room should be kept to a minimum. To help make that happen, we’ve provided our new patient forms here so that you can print them from home, fill them out, and bring them with you to your appointment.
What to Expect at Your First Appointment
When you come in for your first appointment you will be greeted by our friendly front office staff (Carol and Mary). They will help you with any paperwork that is needed and will let one of our audiologists know you are here. You won’t wait long before one of our doctors of audiology comes out to meet you and bring you back for your appointment.
- To begin your appointment, we will have a brief conversation with you to get to know you, understand your hearing needs, and discover what brought you in. We will also go over the paperwork we had you fill out while waiting, this includes your personal and medical history.
- After our conversation we will perform a comprehensive hearing evaluation. The hearing evaluation allows us to diagnose your exact type and degree of hearing loss.
- After the hearing evaluation we will go over the results of your audiogram and answer any questions you may have regarding your hearing loss or overall hearing health.
- After your hearing evaluation is complete then we will discuss your options with you. We will counsel you on the different options there are to manage your hearing loss. If we decide you would benefit from hearing aids, we will go over some styles that we believe would best help you.
- In the state of Connecticut there is 30-day trial period on all hearing aids so you can have time to test your hearing aids at home and experience how they fit and feel in your ears.
